i suggest u use your personal check, if it has your name and current address printed on it. place check on top front page of i821 and bind with paper clip.

buy some paper clips then group your application package in following order.
1) check, i821, tps copy
2) 2pp photo, i765, ead copy, ead renewal letter
3) g1145 form
4) passport old copy, visa copy, new pasport copy, i94
5) bank statement or utility bill or paystubs
place paper clips in different areas so that it wont be too thick. put it all in one envelop.

if you came in F1 like most of people, in question 25.b Authorized period of stay write down D/S with BLACK ink. also while signing at the end of each form sign with BLACK ink only. type the form and when you print it, make sure bar code is printed on the bottom of the page.
