On i-765 number 11: Provide the info of the OPT extension (which is your latest employment authorization application not H1B)

All you need to send is i-821 and i-765 with correctly checked boxes for reregistration. No other documents required for reregistration.
Fee you need to send is $85 for biometrics. No filing fee for reregistration and no fee for i-765 since you are not applying for TPS EAD.

No need to send proof of address if you have moved. Just put your new address in the forms. If you have moved, you should report your new address to uscis using it's online change of address tool.
