i) For payment, either all amount in one or itemized checks/money orders, both are fine.
ii) Do we have to mention anything on the check? Here is the answer I have copied directly from USCIS.
"Make the check or money order payable to U.S. Department of Homeland Security.
NOTE: Spell out U.S. Department of Homeland Security; do not use the initials "USDHS" or "DHS.""

iii) What you have decided to write for In Care Of is fine.
iv) C19 is good for I-765. Even I was confused at first. Learned it from Sajha friends and some lawyers that C19 is the way to go since this is the firs time we are applying for TPS and/or work authorization.
v) Passport is counted as a "primary evidence" by the USCIS. Hence, that should be enough. We are Nepalese. I don't think a lot of us would even care to carry that with us to the States.
