When you apply for MRP, you have to submit a letter stating why you need to have a temporary passport (reason - might say because of family urgency or business travel I guess), separate money order of $20, separate photos (2 copies) and a separate return envelop for temporary passport. Those all should be included with original MRP application and its supporting documents. Once you apply for MRP, you can't separately submit an application/letter requesting a temporary passport. And they don't just send you a temporary passport without you asking for it with fee, letter and photos.
