NY ma ta basdina , but you can download (ofcourse from internet ) the form. Go to a bank with another party from which you seek the power or you are giving your authority to. Every bank has notary service. just sign the papers and get notarized.
This is the basic procedure. But I dont now what do you need it for and it may vary. I can understand, you cant disclose it here for obvious reasons.
