I havenot seen my gf in 3 years....this is my only chance..and i think i lost my passport...please..help...

how to get new passport and what is procedure..how much time..how much cost ...what documents i have to submit?

what if i go nepal with new passport?
 i won't have visa on it? do i have to go to get new visa in nepal embassy?
what about i 94?

i have color copies of everything..but can't find passport....please shed some light ppl
