This is how DSO updatas the employer information in SEVIS sytem while student in OPT and shows the information needed by the DSO to do so in various situations.

-Therefore, for you I assume you could just send email addressing with information needed for New job situation above to your DSO. Better consult with your DSO for the preferred method of reporting changes. SEVP recommends using emails.
