I had a same issue too then I took it to apple store and since i have at&t and the phone was still under apple's warranty, they replace my phone with new one. they said said it is hardware issues and couldn't be fixed there. The problem you have said here is exactly what happen to my phone too so i'm sure it is same issue with your too.
