If you lose your passport, visa and I-94, don't panic; here's what you do:

1. Apply for a new passport from Embassy of Nepal in the US.
2. Apply Form I-102 with USCIS for replacement of the I-94 with $320.
   
3. For the Visa, you have to get a re-stamping.
