She can file a MTR to see if USCIS will re-instate her visa and the cause would be she was pregnant, and went on depression, but to prove these you need all the medical stubs. I am not sure if she can produce such. Even its DSO fault, its really not the issue which is always being overlooked by USCIS. The best thing for you to do is either file MTR, or have her go back home when her hubby be in H1B status. She can get H4 easily from Nepal. That will put all of them in good shape.
