Smegma — Smegma begins to form in the first few days of life as the result of desquamation of epithelial cells in the preputial sac; secretions from preputial glands also contribute. Smegma is carcinogenic, although the specific component of smegma responsible for malignant transformation in men is unclear. Mycobacterium smegmatis, a bacterium present in the preputial sac of about one-half of uncircumcised men, may convert sterols in smegma into carcinogenic compounds [32,33]. In uncircumcised men, the retention of smegma, particularly in the presence of phimosis, may promote malignant transformation by direct contact with chronically irritated epithelium .
