Euta Nepali People had suggested you the same thing in your last thread, and now you're starting over again. Just make sure you write APPLIED in the box, where it asks for your spouse SSN or Tax ID. Make copies of your main and US visa pages, and notarize them from your town/city hall or wherever you find public notaries are. Also follow the instruction to fill out W-7 form. Make no mistake.
