In my opinion sending a filled affidavit form is optional. That's what I would say. I did fill it out but visas got rejected anyway. The most important thing is what Kattu has pointed out. "Ties"
Visitor visa is for non-immigrant purpose. So your relative has to prove that he or she is going to return after the visit and not stay back. Ties can be proven by the property (land/ house) on his/ her name in Nepal, a high earning job, good bank balance statement on his or her name, anything that will make them to come back to Nepal.
