तेयो MRP form मलाई साथीले पठाई देको अनि form मा पहिलेनै NAME & Signature ko thau ma "Lekh Nath Gautam" Designation: Second Secretary लेख्या छ

You should be fine. Your friend must have picked up the form from embassy and they pre fill those two fields these days. Only thing you have to be concern is the finger print and signature shouldn't touch any corner for the box. For peace of mind , you can always call embassy and confirm it.
