USMLE exam/ visitor Visa - ways to stay in US ?

Hi folks , One of my relative has been granted visitor visa for his USMLE exam in US. What are some ways he can stay in US so he can complete all the steps required for the exam? Has anyone done it ? I heard from someone if you find some voluntary work at hospital/clinic you can stay ? But how's that possible on a visitor visa ? Please share if you know anything . Thanks
