people say u only need to send 2 forms, copy of tps approval, EAD copy both sides, letter that came with EAD, $465(your personal check is fine). thats all.

having said that, since this is first re-registration for all, its a guessing game. better to be on safe side. so my suggestion would be

-i821 form. MAKE SURE TO SIGN
-i765 form. MAKE SURE TO SIGN
-g-1145 form
-TPS approval copy, both sides
-EAD copy both sides
-bottom portion of letter that came with EAD. that says its for reapplying new EAD and has bunch of bar codes. they say it really expedites process of ur new EAD
-$465 check or M/O (one check with $465 total is fine. u dont have to make two separate checks. this fee is needed if u are applying for EAD. otherwise its only $85) to US DEPARTMENT OF HOMELAND SECURITY. make sure u WRITE this right.

Now for the OVERKILL.....
- Copy of Passport u came to US with (since its number is the one u put in i821 form). include first page that has expiration date, photo page and visa page.
-copy of new MRP passport. photo page.
-copy of I94 both sides
- 2 recent PP size photo.
- bank statement or utility bill or paystub from june 2015 to now.

I know its an overkill. i rather spend half day and few bucks more now than worry for months later.
