@imjagat, the doctor doesn’t need to list the medications or even recommend for you to go, but it is important to include his name, the hospital’s or clinic’s name, and where. Doctor must mention the name of your relative and his or her condition.

To Whom it May Concern:

I am Doctor Hari Prasad and I am a practitioner at Bir Hospital In Kathmandu,Nepal.  My patient Ram Maya Kumari has been under my care since January 2017 up to the present day.  Ms. Kumari has the following conditions:

  1. Diabetes
  2. High Blood Pressure
  3. Depression
  4. Rheumatoid Arthritis

Under my discretion Ram Maya Kumari is taking the following preskription medicines:

  1. Glyxambi Oral
  2. Amiloride
  3. Celexa
  4. Methotrexate

It is recommended that her son Imjagat Kuikal takes care of her ailments and mental health for a better quality of life for at least the duration of January 15 to March 15.

Thank you,

Doctor Hari Parsad
