 TPS - H1B question.

Hi All,
I am currently in h1b and trying to submit tps form without applying for EAD.
Few questions where i am seeking help:
1) In form I-765 , for question number 11, it is asking for which USCIS location granted my previous work authorization. I have EAD that i have received when i was in OPT extension. And then i am also assuming that i have received EAD while H1b was granted. So which EAD the question is referring to? Is it opt EAD or H1b EAD ? or am i misunderstanding the h1b EAD.?

2) in form I-765, in question 16, what will be my category? is it (tps)(a)(12) or (tps)(c)(19)? how do i figure that out ?

3) for form i-821, do i need to send passport photo ? if yes, how many ?
4) as a proof of nationality, is only photocopy of passport is enough?
5) For the proof of residency, does it have to specific to any time frame OR can it be current ? I am not sure if we need to provide the residency proof during the time when natural disaster occur in our country.

6) i have two passport currently, the old one with visa stamp/ expired and the new with electronic form. do i need to send the copy of both or just the new one?
