They are very particular about the paper size. I did it in 2017 and they rejected the document I sent. But they did send me everything back along with the correct form to fill. Not sure if its the same nowadays as well. I was really annoyed by this. US aayera ni Nepal sarkaar le dukha dina bhanya chadya chaina.

You have to fill passport application online. you get barcode after submission , which you need to take with you while going to embassy.
