 Help with unique F1 visa situation

Namaste everyone,
I did my undergrad and grad from US and worked 30 months post grad (last 3 months were on h1b). I returned back to Nepal 4 years ago. Now, I have been admitted to a doctorate program with instate tuition. I will be done with the program in max 3 years). My I20 shows that I am funding myself, but I can also show my dad's bank statement during visa interview if needed. I have few questions that I am looking for your help answering,
1. Do I need to pay SEVIS again? (My new I20 has different SEVIS numer than my old I20)
2. Do I need my resume for visa interview?
3. When asked about my source of income/work experience, is it ok to say that I worked on OPT or do I have to be specific to mention both opt and h1b?

1) Yes, you must pay the Sevis fee again for a new F1 since it has been more than 12 months since your last Sevis payment(student status).
2) Resume is not required but good to take it with you if you have one.
3) Yes, it is ok to say that you worked on OPT and H1B. Actually, it's better to disclose all info that pertains to your legal work that you did while in the US. If your income specifically came from working in the US, make sure to to mention it. They can see all your visa status and work authorizations anyway so no need for secrecy if everything was done legally.

@galactus Thanks for the info. Yes, everything was done legally and all of my I20 funds came from my US work earning. So, I will be sure to make it known to the visa officer (VO).
Also, do you know if I can take extra documents in a clear bag and hand it to VO only if any of those needed, such as US tax returns, US work related license, resume, dad's bank statement, relationship certificate etc.
Last time the consultancy handled everything for me so I don't remember much. Thanks for taking your time to answer my queries.

You can take all the documents that you think might be helpful but there is no guarantee that the VO will consider any of them unless otherwise asked by him/her. Usually, US universities don't issue an I-20 until one provides evidence of sufficient funds to cover their expenses. So even financial documents are not vetted by the US embassy. Along with your passport, DS-160 confirmation, SEVIS payment receipt, two passport size photos, and form I-20, it's generally a good practice to take your transkripts, diploma, proof of funds, and previously issued US visa documents. Also, it is important to take documents that show you have a strong connection to your homeland and will be returning after you finish your studies. Dual intent is not allowed for a student visa so you must convince the VO that you plan to return "home".

@Galactus Thanks again . Hopefully last couple questions. On DS160 form (1) have you been ten printed? _ do I answer it Yes since I had mine biometrics done for TPS when it was granted 1st time (2) Has anyone ever filed an immigration petition on your behalf with USCIS?_ do I answer it Yes since I was on h1b.

My online research shows Yes and No for 1 and 2 respectively with some ambiguity, so seeking additional feedback.
