@westcoastlad: Taaremam Pasa:These are the forms that you need to fill and submit with application fee and may be a picture for I-765 as well. I believe you have to fill the following form: - I 765- https://www.uscis.gov/i-765 - I-821- https://www.uscis.gov/i-821 (if you have reapplied may be you don't have to apply again please check this on your end) - G-1145- https://www.uscis.gov/g-1145 The link above do have have instruction to fill these forms.There might be resources in google and youtube as well.
