 tourist visa to Nepal

I am now a Citizen of Canada and I'm visiting Nepal. I think I can apply the the nepali visa at the website:
http://www.online.nepalimmigration.gov.np/tourist-visa

This website doesn't say what is the visa fee. Do I have to pay the fee in $ or Rs.?
However I don't see any guide for filling the form, e.g. what is the photo size required?
Does the photo need to be nepali passport size or US passport size is ok?

Also looks like I have to print the slip after submitting the form and take it along with me and give it at TIA. Is this how its supposed to work?

If you have canadian passport, they will take care of you in TIA. Why you want to go through long process over here.
