Traveling with a newborn to Nepal

Hi,

I'm basically asking this for a friend. His wife had a baby in the US. While he is in Nepal, she wants to take him back.

He showed me a form that needed notarization. Does he really need to fill out the form? Shouldn't his wife's info suffice?
