I'm referring old company below as the company that you are leaving. Talk to your lawyer of the old company and/or new company. In general, your new hiring company should take care of these stuff and will guide you through it. There is always an exception and lwayers know how to deal with it. Provide the pay stub of the month before your unpaid leave ( if applicable) Provide your documents reflecting your unpaid leave. You might get this from your old company. ~GooDLuck
