Dilemma: Visa renew in US vs new visa in Nepal

my mother-in-law is planning a visit to the US next year. She has US visa from previous visit which expires in May 2011. But she plans to come in March and stay untill June-July. Will it be better to get new visa in Nepal, or come to the US and then file for Visa extension? She has visitor visa. any suggestion is highly appreciated. Thanks.
