If DHS knew you didn't know how to write a statement for OPT would you think DHS would approve your OPT? No offense - you should try by your own or talk to your internation advisor.

You don't need to write any statement letter to DHS for OPT. i got my OPT without writing any statement . i did extend my OPT without writing any statement. i don't know if the rules have been changed recently. but i guess not.............. you just need to send I-765 form, New I-20 Bank cheque Photocopies of all I-20's issued to you since you first entered in F-1 status. Include all pages Photocopies of the identification page of the passport Photocopies of both sides of the I-94 card Photocopies of both sides of any previously issued EAD cards if have any Two recent photos of yourself.
