Go to nearest police office and take your finger print and charge you some money. Then send that finger print of 10 fingers to FBI with $18 money order each person. They will send that back to you with stamp of no record found. Finally send that to the Canada in the given address. GOod Luck.

Request one with your nagarikta, passport, visa and I-94 copy to Police Headquarters in ktm. They will issue it. That's how I did mine.
