When are you planning to visit? if it is during dashain time then I would suggest you to wait until Uscis publishes the updated version of Federal Notice. I wrote a cover letter that I needed to go visit my family for Dashain break and didn't provide any other documents. They approved my AP for Multiple Entries.

I am planning to visit in sometime in July before school starts for me. A cover letter explaining somewhat fragile health of my grandfather ( he is 84 and always ill), would that suffice?

If you have doctors note and make sure the note is in the cover letter of the hospital. and yes cover letter too

@Situ Manandhar.....What kind of AP do you get? Is it a Multiple entry on your TPS EAD card or a separate sheet of paper?

if you apply AP through TPS you will get it on paper, the combo card is for those who are in the process of adjusting their status such as green card via marriage and so on.
