Will USCIS accept handwritten i-693 form from civil surgeon? Or does it have to be computer typed as i am doing for other USICS forms? Additionally, When i asked Civil surgeon if i will receive copy of i-693 form after its completed, she replied saying entire i-693 form will be in sealed envelope and i will only get copy of Part-9 "vaccination record page" for myself as a copy. Are we allowed to request the entire copy of i-693 form after its completed by civil surgeon or not?

It can be handwritten. Nope you are not required to get a copy of what they seal.
