Answer. #10 - Yes, correct. BUT on this #10 you have a choice to write your A# instead of i-94. You may have your A# on top your name of your EAD attached letter. Remember you need to tear-off portion of this EAD attached letter to speed your application for an extension. #11. Since have work permit now, you have to write "YES". For USCIS office: Look for the EAD card# on the front of your EAD card. In the example below, the EAD card number is SRC0000000773. The first three letters are an abbreviation of the USCIS office that handled your application. Here is a list of the abbreviations for USCIS service centers: CSC - California Service Center EAC - Eastern Adjudication Center IOE - USCIS ELIS (e-file) LIN - Lincoln Service Center MSC - Missouri Service Center NBC - National Benefits Center NSC - Nebraska Service Center SRC - Southern Regional Center TSC - Texas Service Center VSC - Vermont Service Center WAC - Western Adjudication Center Date This is the date that your employment authorization began. You’ll want to use the date next to the “Valid From” label on your card. In the example below, the date is 01/01/80. If your previous application was not approved, use the date on the Form I-797, Notice of Action, that you received.
