Send email to Nepalese embassy, they will send you the form. For mine, I got the form in two days after I sent email. Naya passport chahi blank nai aauchha, saath maa purano passport pani firta aauchha. Illegal harule passport banauna pathauda kehi problem hudaina mero purano passport maa visa nai thiyena.
