It's easy... you don't need guidelines to fill the form.. just print two copies of passport application form from... http://www.nepalembassyusa.org... and fill in both copies.  Filling the form is easy....just fill in whatever is asked  The documents required are shown on the site listed above. Yes, it's like applying for new passport, and you need to send your old passport and/or nepalese citizenship card. You might wanna use fedex or ups to mail your application docs to embassy and also sent the paid return envelope (fedex or ups) so that they will mail your new and old passport back. The embassy will send your documents to Nepal for new passport and once it receive them, it will mail to you using your prepaid return envelope. So it would take a while. when i did, i make sure its not delayed i usually call them after 2 weeks every alternate days so that they wont delay and i feel you should also do that

Where do you guys have your photo taken for 45mm x 35mm size? Can we have that size photo taken  in Walgreen or similar stores? Do I need to send the original citizenship card?

yes u can send both pictures taken at walgreens and cvs pharmacy
