Bro, regarding your question # 2, 821 ko instructions maa clearly bhaneko cha you do not have to submit supporting documents like you did for the initial registration. Please read below.
For question # 1, I'd still put H1B as your current status. Or better yet, you can say "H1B (currently on TPS)".
