Correct, if i130 is not approved, the fee for other applications will not be refunded. The filing fees are non refundable.
you can apply i130 and rest of the other applications concurrently only if you r a US citizen. If u r a GC holder, you can apply i130 only and not others since visa number is not available for spouse and kids of GC holder.
Medical examination can be done anytime. Medical certificate by civil surgeon is valid for 1 year from the signed date to apply for i485 and doesn't depend on i130. u can plan accordingly.
