- Form I-539
- (fee) personal check or money order payable to DHS
- Original I-20 issued by University
- Form I-94 copy or original (please also include I-797A, Notice of Action)
- I-134 form with your sponsor’s bank statement or letter
- SEVIS fee receipt
- Personal statement explaining why you would like to change the status
- Copy of passport and visa
- pay stub may required but it depends on DHS officer (in my case they didn't ask)
