Dude, i'm afraid that most likely they will not consider you as a medical student any more since you are already into your internship phase (even though you have not graduated yet). This is the trend but i might be wrong. If you are considering it very strong, talk directly with someone from the medical school you are applying into and try to convince them that you are still a medical student and have not started your residency yet. Otherwise i would suggest you opt  the regular USMLE pathway and consider applying for research/extern/observer position. Overall the results are not that depressing for Internal Medicine. What speciality you wanna go into? Hope you are not thinking about geting into Neurosurgery, ENT or Orthopedics. If for Internal Medicine you just need to prepare yourself, get good board scores, have some patience and you will be just fine. 

Honestly, in your case, i think you should just choose the usual route and forget about doing a rotation here. Take  the boards, score high, good if you get some clinical experience in the US and probably  be able to start residency in June 2012.
