I am not aware of any nursing requirement, nor I know anyone who went through this procedure. But I feel like I need to give some input,  I know about immigration stuff. I am just trying to correct some information floating around and may not be useful in your context.

1 St thing about above post, if the guy is GC holder, your sis. cant come to U.S immediately as his dependent.  She might end up waiting 4/5 yrs to come to U.S. There is other way around like coming on student visa, or K visa. And these visa types have limitations.
2nd , if guy has phd or phd student doesn’t mean they can apply national interest waiver (NIW), and get G.C in a while. He need to be qualified and believe me it is not as straight as may people think. He need to convince U.S government that giving him G.C is in interest of U.S government. But probably, they will qualify for EB2 and get GC in few yrs.
3rd I personally don’t know any thing about licensing exams etc but some one mentioned above, that getting IELTS score is very tough , but I saw in website (http://www.cgfns.org/files/pdf/req/vs-requirements.pdf) that the requirement for RN is 207 for TOEFL, which is way too low   then admission requirement of most of the U.S schools. So, getting that score might not be big deal. ( But I could have misinterpreted and may be wrong on this. Someone can correct me)
4th if you come as dependent of H1B, she don’t need to change her status just to go to school. She can join school as H4. But there are other stuff like GA, RA and instate tuition fees. But also H4 has limitations.
Finally, as per my experience career and Immigration in U.S is very complicated these days, but it might work for someone easily. So, basically it is case my case basis. Taking risk depends on many thing like passion, dream, kind of life u want to live, driving force for taking decision etc…etc….
