that's interesting. what kind of insurance is your friend under - university, private, etc?
as far as I know (from my insurance company's policies), they treat both emergency and chronic diseases if the disease/disorder was diagnosed overseas. even if the case is then transferred to US, they take responsibility of it. however, if outside US, they pay only 50% of the charge but inside, they pay 80%. if the disorder is very acute (like requiring brain surgery, heart surgery, critical organ transplantation), it is free. BTW, this health plan I'm under is the one provided by my university.
