what are the details that you have to provide in the AP form? Especially regarding part 4(purpose of trip)? what kind of supporting evidence that needs to be provided?
is simple 'family visit" would be enough ?

If you want to travel ASAP then you need to describe and also need to submit the letter which reflects that your presence in family is very important. Letters has to be sent from your family. This is my case in last year and I was approved AP within a day. You even don’t have to mail. You can go to local USCIS office and submit the document package. If you don’t have rush to travel then “visiting family” would be enough but it takes around 2.5 month to deliver your AP in the mailing address which you have given. GOOD LUCK.

For Family visit:

Write a letter stating the dates you want to visit and why are you visiting them. I wrote that I would like to celebrate Dashain and Tihar with my family Also i mentioned i will travel other countries to visit my relatives just to let them know that I will need a multiple entry AP. But if you have immediate family emergency you can either obtain a doctors note from nepal and get AP in one day but that AP will be valid for only one entry!
