Thank U for the replies. Actually I tried downloading couple of times. Here is the issue. When the downloaded form is saved in the computer and opens it, it has bar code but when I fill the form itself that is on the website hasn't. I m not using MAC PC.

@pshres1 You are correct, only when the form is downloaded and opened via adobe acrobat or similar pdf software, u see the barcode. So always download the form and fill in, and see the changes in the barcode as u start filling in just for confirmation.

Always fill out the downloaded form. Save in-progress file, print/export to pdf when it's done.
