Hi all, quick question on # 7, 10, 10B, and 11B from the application form above.
# 7 : Do I type "/" as well in the Citizenship No? The # is in "XXXX/XXXXX" format
# 10: Assuming Old Passport No is still the current passport no. ?
# 10B: Place of Issue of passport: KATHMANDU? Just asking for the district?
#11B: Municipality/VDC Does "K.M.C" work? Five characters in the form.

Also, can anyone please validate that I have to send the application to the following address, and not to the Embassy in DC, since I'm applying from NY? Thanks!
