So our PM is equivalent to Indian Minister of State (rajya mantri) ?
India knows who the prime minister of Nepal is and knows how he was installed (sorry, i could not find better word to describe this person) in this position and knows who could be the right person to welcome him in the airport. Baru Girija le Nepal ko ijjat dhaneko thiyo ek patak prime minister bhayera India jada after Jana Andolan II; Indian prime minister Man Mohan Singh himself went to the airport to welcome Girija that time.

I am wondering if there is a difference in protocol when its a "state visit" vs "goodwill" visit.
जहाँ सम्म मलाई लाग्छ, भारत सरकार को चलन अनुसार कुनै पनि बिदेशी राष्ट्र प्रमुख वा सरकार प्रमुखको सरकारी भ्रमणमा हवाइ अड्डामा बिदेश मन्त्री ले स्वागत गर्ने गर्छन्।
राष्ट्र प्रमुख भए राष्ट्रपती भवनमा भारतीय राष्ट्रपतीले औपचारिक स्वागत गर्ने तथा गार्ड अफ अनर लिने दिने चलन छ । प्रधानमन्त्री को भ्रमण भए प्रमले आफ्नो कार्यालय प्राँगणमा औपचारिक स्वागत गर्ने गर्दछन ।
गिरिजा प्रशाद कोइराला को भारत भ्रमण मा चाँही प्रम मन मोहन सिंह आँफै हवाइ अड्डा आइ स्वागत गरिएको थियो। त्यो परम्परागत नभै एक अर्काको सम्मान र आत्मियता को रुपमा लिनु पर्ने हुन्छ ।

You may be right.
May be because of goodwill visit not a state visit.
