Shortage of Medical Doctors in Nepal
Nepal is experiencing a severe shortage of medical doctors, not so much in Kathmandu City, but in the towns outside of the valley. I am not talking about remote rural areas. Even relevantly urbanized Sunsari District in Eastern Nepal does not have sufficient medical hands. Take a read of the article, from Kathmandu Post, attached below.
Now contrast this with the overwhelming number of medical doctors who have recently come to the US from Nepal. Some have put this number to more than 20 thousand, though I think the number is close to couple of thousand. These Nepali doctors are here to give USMLE exams that will allow them to take up residency in a US hospital, after 3 years of which, they can get a MD job earning high salary and US Green Card.
I am not blaming doctors. Everybody has the right to pursue happiness and live the life as they see fit. But the problem in Nepal should also not be ignored. If the doctors want to flee Nepal, there must be a reason. Of course, higher pay and better lifestyle are obvious reasons. But, given that there is so much demand for doctors in Nepal itself, the government should take necessary steps so that those doctors will have less reasons to emigrate.

Half of doctors in eastern region absent

DHANKUTA, Jan 18 - Half of the quota for doctors in district hospitals and Primary Health Centers in the Eastern region are vacant.
According to Regional Health Directorate here, only 44 out of 95 doctors sanctioned by the government for 16 district hospitals and PHCs are working at present. Of the total, 40 seats are vacant, while 11 have gone for study leave. Similarly, only four out of 51 PHCs have doctors. Of those working doctors, some go for training and some to their homes, a source at the directorate said.
There are no doctors in Ilam, Dhankuta and Panchthar district hospitals. These hospitals have quota of three doctors and a medical superintendent. "It's hard to have even a single doctor in the district hospital, forget about filling other quotas," an employee at the directorate said.
Sunsari hospital, with a quota of four doctors, has only one working there currently. None of the PHCs in the hilly district have seen doctors, while they can occasionally be found in the terai belt.
Similarly, about half of the posts in Koshi zonal hospital are vacant. Of the 51 doctors, only 27 are working in the zonal hospital.

कुरोलाइ यसरी बुझम नानु जी । पहिला पहिला हाम्रो जमाना मा पो राष्ट्रले पढ्न पठाउंथ्यो डाक्टरि र अभियन्ता अनि राष्ट्र प्रति अलिकति भए नि जिम्मेवारि महशुस् हुन्थ्यो। अहिले को जमाना फरक् छ। भन्ने हो भने हाम्रो देशमा चाहिले भन्दा बढि डाक्टर् साहेब् र इन्जिनियर् महोदयहरुको उत्पादन् भै राख्या छन्। बार्षिक ६ हजार् इन्जिनियर् र १५ सय चिकित्सकहरु नयाँ थपिन्छन्। र पनि डाक्टर् हरुको अभावमा देशका अस्पतालहरु अनाथ् बनेकाछन्। यो कुनै आश्चर्यको घट्ना होइन। एक जना डाक्टरि पढ्न करिब् २० लाख् रुपैया खर्च लाग्छ। कहिले उठाउने २० लाख् लगानि? नेपालि अधिकृत स्तरको कर्मचारिले बार्षिक १ लाख् पनि तलब् पाउंदैन। जिन्दगि भरि नै लगानि न उठ्ने भए सि प्राय बिदेशिएका हुन् नया डाक्तर् साहेब् हरु। हुन पनि सबैका छोर छोरि लाई डाक्टर् इन्जिनियर् बनाउनै पर्ने मानसिकता छ, के गर्ने?
त्यसैले भ्रस्ट्राचार् नियन्त्रण् गर्न को को भ्रस्ट छन् पत्ता लगाउन चाहिने न चाहिने प्रमाणको खोजि गर्नु नै पर्दैन। नेपाल् भित्र र बाहिर डाक्टरि र इन्जिनिरिङ्ग् पढ्ने हरु को सुची अध्ययन गरे पुग्छ। महिनाको १० हजार् तलब खानेहरुले छोराछोरि हरु बार्षिक् ४-५ लाख खर्च गरि कसरि पढाइ राख्या छन्, स्पष्ट हुन्छ। सबै को गाड् धन् त हुदैन नि कसो?

Though I feel bad about the brain-drain syndrome, it is a normal economic process. Mobility of labor is nothing new especially in this age of globalization. Similar to the docs, we too arehere in the US seeking a better life than the one we had back in ktm.
However, I believe in Nepal's internal case, the shortage of docs outside ktm could be attributed to the rampant maoist activities there. A doc surely earns more than the local average income that undoubtedly attracts the attention of the damn commies and other local free-riders...If you were a doc, for the safety of your/your family's life, would you still remain in those areas?
Now that peace has "allegedly" arrived, lets hope that in due time we have the docs returning to the eastern regions.

धनकुटा, माघ ३ - पर्ूवाञ्चलका १६ वटै जिल्ला अस्पताल तथा प्राथमिक स्वास्थ्य केन्द्रमा ९५ जना चिकित्सकको दरबन्दी भए पनि ४४ मात्र कार्यरत छन् ।
क्षेत्रीय स्वास्थ्य निर्देशनालयका अनुसार तीन वटा अञ्चल अस्पतालबाहेक ९५ जना डाक्टरको दरबन्दी छ । ती मध्ये ४० वटा दरबन्दी हाल रिक्त छन् । ११ जना डाक्टर अध्ययन तथा असाधारण बिदामा बसेकाले ४४ जनामात्र कार्यरत रहेका हुन् । ती मध्ये पनि कोही तालिम तथा घर बिदामा जाने गरेकाले थोरै डाक्टरमात्र काममा रहेको स्रोतले बतायो ।
धनकुटा, इलाम र पा“चथरका जिल्ला अस्पताल यतिबेला डाक्टरविहीन नै छन् । सबै जिल्ला अस्पतालमा एक जना मेडिकल सुपरिन्टेन्डेन्टसहित तीन डाक्टरको दरबन्दी छ । ती दरबन्दी कागजमा मात्र सीमित रहने गरेको गुनासो स्थानीय बासिन्दाको छ । 'जिल्ला अस्पतालमा एक जना डाक्टर त ह“दैन कहा“बाट तीन जना पुग्नु -' निर्देशनालयका एक कर्मचारीले भने ।
चार डाक्टरको दरबन्दी रहेको सुनसरी अस्पताल अहिले एक जनाले धानिरहेका छन् । पर्ूवका ५१ वटा प्राथमिक स्वास्थ्य केन्द्रहरूमध्ये चार केन्द्रमा मात्र डाक्टर रहेको निर्देशनालयले जनाएको छ । प्राथमिक स्वास्थ्य केन्द्रमा एक जना डाक्टरको दरबन्दी रहने व्यवस्था छ । पहाडी जिल्लाका कुनै पनि केन्द्रमा स्थापनाकालदेखि नै डाक्टर पुगेका छैनन् भने तर्राईमा आकलझुकल एकाध महिना डाक्टर जाने गरेको स्रोतले बतायो ।
स्रोतका अनुसार लोक सेवा आयोगले डाक्टरको पदपर्ूर्ति गरे पनि आयोग पास गरेका डाक्टरहरू सुगम क्षेत्र पाएमात्र जाने नत्र जागिर खानै नजाने गरेकाले यस्तो समस्या आएको हो । मुलुकमा ठूलो धनराशि खर्चेर मात्र डाक्टर पढ्न सक्छन् । त्यसैले गाउ“को परिवेश नै नदेखेकाहरू दर्ुर्ुगम क्षेत्रमा जानुभन्दा सरकारी जागिरे नबन्नु नै बेस ठान्छन् ।
'सरकारले निर्धन अभिभावकका छोराछोरीहरूलाई निःशुल्क छात्रवृत्ति दिएर डाक्टर पढाउन सकेमात्र दर्ुर्ुगममा डाक्टर बस्छ' ती कर्मचारीले भने- 'गाउ“लेहरूका लागि डाक्टरको सेवा पाउनु आकासको फल आ“खातरी मर जस्तै छ ।'
ती कर्मचारीका अनुसार धेरै पटक माथिल्लो निकायस“ग डाक्टर माग गरी पठाएकोमा माथिल्लो निकायले कुनै चासो देखाएको छैन । डाक्टरहरू सरकारी अस्तालमा काम गर्छु भन्छन् तर निजी क्लिनिकअगाडि बोर्ड झुन्ड्याएर राखेका हुन्छन् ।
जिल्ला अस्पताल र प्राथमिक स्वास्थ्य केन्द्रमा मात्र होइन अञ्चल अस्पतालको अवस्था पनि दयनीय छ । ५१ जना डाक्टरको दरबन्दी रहेको कासी अञ्चल अस्पतालमा २७ जना डाक्टरमात्र कार्यरत रहेका छन् । पर्ूवका तीन अञ्चल अस्पतालमध्ये मेची र सगरमाथा अञ्चल अस्पतालमा समेत दरबन्दी अपुग रहेको स्रोतको दाबी छ ।
