It should have been the most polluted City not Country. Kathmandu or dhulomandu as a city or valley makes sense not the whole nation Nepal in general. 
Once you reach a higher altitude or go to remote places like Rara, Upper Mustang, Shey Phoksundo, Gosaikunda in Nepal,  you won’t find such crisp and clean quality of air anywhere else in the world. 

Very Misleading information by western media.

Very plausible if you look at the skyrocketing numbers of cancer patients and kidney failures in Nepal. Apparently the statistics seemed to be based on the specific city rather than the entire country. Also, the list keeps changing based on when the study was done but Kathmandu has been in the list consistently. Maybe its time to flock back to your villages if you want to live longer life.

Situated in the towering Himalayan mountains, this multiethnic, multilingual, multi-religious country is home to 31 million people.

Air quality monitors in its capital Kathmandu, home to 1.5 million people, shows that increasing emissions from vehicle exhaust and industry are harming health, particularly in the winter.

Farming practices such as brush fires and wood cooking in rural regions are also taking a toll.

One of the most alarming effects of Nepal's environmental decline is seen in the changes to nearby mountain glaciers.
