India is so big you can create itineraries for months. Some ideas:
Big religious temples.
Goa
Taj
Health Treatments of parents/relatives etc etc
I hope you are planning for 1-2 months not half a year.

But yaha farkesi port of entry whereever .. ok jfk or O'hare ma how would i convince them ki i was in india for 20 days in india bhanera.. would they ask for

As Max said lots of people have used India before. You make sure to use citizenship in the Indian airport. Indian authorities some time like to hassle nepalese and will ask for passport but make sure not to give them.
Get seperate round trip flights. One us-ind and one ind-nep.
During POE if you don't have any nepalese stamp they can't proove you went to Nepal. Medical reason works best as a reason why you stayed in India for a month.
Try to get flight from Abudhabi so you clear POE there and enter the states as a domestic flight. Less hassle.
I know you're nervous but you seem to be repeating your doubts over and over again.
Tldr. Use India. No stamp at Indian Airport. Abudhabi for POE.

usanep, not a legal advice, from what I have read, you can create a travel authorization for your child to be with relatives. Its a bit of an hassle, but you could have someone come to India and fly along with you or in another flight same day or ever earlier than you guys. We have a open border but authorities may ask to see your citizenship.

@ruler brother use some common sense, how hard is it for you for you to come up with itinerary, already told you use medical reasons, say you stayed at one place through the trip. use google to find a hospital name and do some quick searches of things to do around that area. If your'e asking if agents will ask about whole itinerary, then i am not qualified to answer that. Some people are lucky some arent.Usually they wount if you can tell them in general what you did and where you were.
A final suggestion for you is erase your phone before you return and dont store any pics of stuff that could tell you have been to nepal.
