it means, the company will present your CV to client. If you apply for same job from different company, you cannot sign that form again, it will disqualify. If this is the first application, and you are good with everything, you can sign it.
Your signature is required to send your resume to client.

I am not sure on which technology are you applying for and how much they are offering, some detail is required to explain different scenario. There is no risk, later if you don't like the job, you can say , I got another better offer.
i was most concern about contractual obligations. its an offer of $xx/hr on W2 without benefits. but it is going to be double than what i am making hourly now. i am not so worried about benefits since i have through my spouse. i probably won't get paid for holidays and vacations. not sure about this its new to me.
then go ahead, no problem. this step is to submit your CV to client for interview/selection process. This doesn't mean you are obliged to work for them. This will not effect anything.
